Privacy and request handling
Understand what the request form collects, what the calendar means and how to send only the information needed for a response.
Your request
The form prepares a message containing the information you enter. On the custom-domain PHP hosting version, it submits that message to the site email through the hosting mail system. On a static preview, it prepares an email draft for you to review and send yourself. The preview does not silently send a request. The form does not take payment or create an account.
Calendar preferences
A selected date and time are preferences, not proof of staff availability. They are included in the email so the recipient can reply. No confirmed appointment is displayed until a separate confirmation is received. Avoid sending urgent medical or emergency requests through this channel.
Minimise personal information
A street address is optional at the enquiry stage. A ZIP alone cannot establish city jurisdiction; an address may be needed before confirming a location-specific service. Do not include identification documents, passwords, payment-card information or sensitive health records. Search input is processed locally in your browser; it is not submitted to an analytics provider.
Hosting and email
The web host may keep ordinary security/access logs. The PHP delivery endpoint temporarily stores a hashed network identifier and request timestamps for abuse rate limiting, not the enquiry body. Email messages remain subject to the mailbox provider’s storage and security settings. No marketing-list subscription is included. Contact the site email to ask about an existing request or request deletion of correspondence; no automated deletion guarantee is made.
